Piezoelectric Bone Surgery in Dental Extractions and Implants

Piezoelectric surgery uses ultrasonic vibrations to cut bone with extraordinary precision while preserving soft tissue, revolutionizing dental extractions and implant placement.

Piezoelectric surgery uses ultrasonic vibrations to cut bone with extraordinary precision while leaving soft tissue largely unaffected. This technology represents one of the most significant advances in surgical technique available to oral and maxillofacial surgeons today.

How It Works

Traditional bone cutting relies on rotary instruments (drills and burs) that operate through mechanical friction. Piezoelectric devices, by contrast, generate ultrasonic frequency vibrations (between 25,000 and 50,000 Hz) at the cutting tip. These micro-vibrations selectively cut mineralized tissue (bone) while stopping at contact with soft tissue such as nerves, blood vessels, and the sinus membrane. This is known as the “soft tissue selectivity” of piezoelectric cutting.

Applications in Oral Surgery

In wisdom tooth extraction, piezoelectric instruments are especially valuable for removing impacted third molars adjacent to the inferior alveolar nerve. The ability to work precisely without endangering the nerve reduces the risk of post-operative numbness. For dental implant site preparation, piezoelectric osteotomy allows precise bone modification with minimal thermal damage and reduced risk of membrane perforation in cases near the sinus floor. In sinus lift procedures, the Schneiderian membrane (lining the maxillary sinus) must be carefully elevated without tearing. Piezoelectric instruments significantly reduce perforation rates.

Patient Benefits

Reduced risk of nerve injury, better soft tissue preservation, less post-operative swelling and discomfort, and potentially faster healing are the primary patient benefits of this technology. Not all cases require piezoelectric techniques, but for complex cases involving proximity to critical anatomy, it represents the current gold standard in precision surgical care.
