We are pleased to offer Same Day Surgery for selected patients. To schedule efficiently and accurately, please ensure the following steps are completed before your patient’s appointment.
1. Complete and submit the online Referral Form — accessible via Refer Patients.
2. Send a recent panorex taken within the last year — electronically to info@stevebureauoms.com or by mail to our office.
Complete the online Patient Registration Form — accessible via Patient Registration.
All forms are reviewed promptly, and your patient will be contacted shortly after. Please note that not all patients are candidates for Same Day Surgery — if this is the case, a separate consultation will be scheduled.
